Most major accidents involve a change that somebody did not think was a change. The course covers what counts as change (equipment, chemicals, procedures, organisation, temporary and emergency changes), how to scope the review so it is proportionate, and how the PSSR confirms that the change was built as designed, procedures and training were updated, and PHA actions were closed. Participants judge twelve real cases and run a tabletop PSSR on a modification package.
