CauseIQ is incident investigation and root-cause analysis software. It structures the investigation — evidence, timeline, causal factors, fault tree, barrier analysis — and links every finding back to the PHA safeguards and PSM elements it affects, so lessons actually change the system.
An investigation that ends at 'operator error' or 'valve failed' has not found a cause. CauseIQ drives the analysis to management-system causes using recognised techniques and keeps the evidence trail an auditor or regulator will ask for.
Because it shares the safeguard library with CeraPHA Studio, a failed barrier in an incident is the same object as the safeguard credited in the HAZOP — the PHA can be revalidated on the spot.
